The challenge
Facilitating collaboration among diverse stakeholders in the elderly care landscape of South Limburg. The goal was to create a transparent and accessible tool that supports regional cooperation, knowledge sharing, and resource optimization.
Our approach
We designed and implemented an innovation funnel and platform tailored for eight professional partners within ZorgConnect Zuid-Limburg, specific the Community-Based and Informal Care team (WIJZO/INZO), with potential expansion to municipalities, welfare organizations, housing corporations, and care providers. The platform enables:
- Clear insights into ongoing community and informal care initiatives.
- Efficient collaboration, preventing duplication and maximizing resources.
- Knowledge sharing to create collective intelligence and strengthen partnerships.
- User-friendly navigation, with search filters and visualization at municipal and neighborhood levels.
The impact
The platform enhances transparency, streamlines collaboration, and fosters regional innovation. It provides structured access to community-based and informal care initiatives, allowing stakeholders to align efforts and drive meaningful change. While currently embedded within ZorgConnect Zuid-Limburg’s infrastructure, further public accessibility remains an ongoing discussion.
